1.  If you are refilling your prescription early (you have more than 25% of your medication according to the directions on the bottle), please indicate why:

* Going on Vacation

* Lost Medication

* Doctor has changed the way you are taking the medication.  Please note how much of the medication you are currently taking daily.

* If other, please explain.

2.  Also include comments or special requests.

E-Mail Request

Requests will be read daily (except Sundays) at 9:00AM and 2:00PM.  If your prescription has refills and is e-mailed by 9:00 a.m., it will be ready by 12noon.  If your prescription has refills and is e-mailed by 2:00PM, it will be ready by 4:00PM.
